DescriptionIdeal candidates have at least 2 years of experience in care or case management and in-depth knowledge of the field of aging and care management services; are passionate about working with older adults and adults with disabilities; and are tenacious and creative problem-solvers with an optimism about what is possible for our client population.
Compensation is based upon training, education and experience.
Serving the greater San Mateo, Santa Clara and San Francisco.

) Comprehensive Medical, Dental & Vision plans HSA and FSA plans 401K Paid time off Annual bonus incentive Employee referral program REQUIREMENTS Licensed MSW, MFT, OT, RN, MA Gerontology or related field highly desirable Experience in care or case management (minimum 2 years) Knowledge of local eldercare resources (preferred) Demonstrated ability to provide sound professional guidance, grounded in in-depth knowledge and experience in the field of aging and care management best practices In-depth knowledge of all aspects of the aging process and conditions affecting older adults and adults with disabilities, including memory care Impeccable attention to detail and strong follow through Integrity, honesty, and commitment to high standards of excellence and professional ethics Commitment to working collaboratively with a multidisciplinary team Excellent listening skills and communication skills (written and verbal) Ability to prioritize work and manage time effectively Proficiency in Microsoft Office Suite, Google Drive, Google Chrome Availability all weekdays and some evening/weekend on call rotation (approximately every 6-8 weeks) Valid California driver's license, insurance, personal transportation (care managers work from home with some travel to clients' homes) RESPONSIBILITIES Conduct initial phone calls with prospective clients and community members, demonstrating a holistic understanding of client needs and clearly articulating how Sage is equipped to meet the needs.
Conduct comprehensive in-home assessments.
Draft written summaries of findings and recommendations in language that is clear and comprehensible to clients' responsible parties, and that are useful in formulating or updating a client's care plan.
Develop clear and concise, goal-oriented care plans upon completion of the initial client assessment to be shared with the client and/or responsible party for review and developing a follow up plan.
Implement all aspects of the care plan, including following through on action items, identifying barriers to follow through and achievement of goals, and updating and modifying as needed.
Navigate family dynamics to maximize success in achieving client goals.
Communicate and collaborate regularly with family members and appropriate professionals to advocate for clients' needs.
Timely and thorough completion of required documentation of services Provide on-call services as part of an on-call team rotation.
Contribute to the overall support and functioning of the Care Management team through active participation in meetings and providing coverage of cases as needed.
Provide billable services to clients.
